The Delhi Community Health Centre (DCHC) is a community based not-for-profit organization dedicated to providing quality, patient-centered health care for individuals and families residing in Delhi and surrounding communities.
A variety of health care services and providers are available through the Centre. Housed within the Centre, the Delhi Family Health Team (FHT) provides access to family physicians, a nurse practitioner, registered nurses, social worker, mental health workers, pharmacist and mid-wife. A community Walk-In Clinic is offered and, also available, are additional family physicians and specialists who provide care via their private practices.
Supporting individuals who are accessing their primary healthcare through the Centre, in addition to supporting the broader community, a pharmacy, community lab and ultrasound services are available on site, as are a variety of other community services intended to enhance the overall health and wellness of men, women, children and families.
A secondary site can be found in Port Rowan.
The FHT has recently received funding from the Ministry of Health to launch a Chronic Non-Cancer Pain Management Program. This program will serve Haldimand-Norfolk and Brant Sub-regions of the Hamilton Niagara Haldimand Brant Local Health Integrated Network (HNHB LHIN), with sites in Delhi, Dunnville and Brantford and will provide interprofessional primary care interventions to support individuals with reducing their prescription opioid use.
The interprofessional primary care team will be comprised of Nurse Practitioners, Registered Nurses, Social Workers and Physiotherapists, who have expertise in pain management. This team will be working alongside of a variety of physicians who specialize in specific areas of pain management and opioid alternative therapies.
An immediate need exists for one full-time Social Worker. The full-time Social Worker will be dividing his/her time between Dunnville and Brantford.
The Social Worker with the Chronic Non-Cancer Pain Management Program (CNCPMP) works alongside of the interprofessional primary care team, providing assessment, counseling/therapy, psychosocial health education, crisis intervention and advocacy for individuals who are living with chronic pain and are undergoing changes in the treatment approach to their pain management. Therapeutic interventions are short term.
The Social Worker:
- Is responsible for providing assessment and counseling/therapy for patients who have historically been prescribed opioids for management of chronic non-cancer pain and are undergoing opioid titration or engaging in alternative pain management therapies.
- Is responsible for working collaboratively with the patient and interprofessional primary care team to develop a personal care plan, including short and long-term patient goals, and to provide support and interventions in alignment with the same.
- Is responsible for the delivery of services in a 1-1 setting or through group facilitation.
- Is responsible for facilitating system navigation and the provision of information and referral to appropriate services.
- Reports to the Executive Director and works closely with other members of the interprofessional primary care team.
- Advanced degree in social work, nursing or psychology with additional training in crisis management and suicide prevention
- Member of a regulatory college (e.g., College of Nurses of Ontario OR Registered College of Social Workers and Social Service Workers OR Registered College of Psychotherapists of Ontario)
- A minimum of two years’ experience working with individuals living with chronic pain
- Knowledge and proficiency in current, evidenced-based methods and practices of social work in primary care settings, with an emphasis on chronic disease management and self-management, health promotion and risk reduction
- Understanding of social determinants of health and their impact upon health and well-being
- Knowledge of relevant privacy and health care legislation and community health and social service resources
- Strong clinical, organizational and interpersonal skills

- A positive and professional attitude
- Excellent time management skills with an ability to multi-task and effectively set priorities

- Multilingualism (e.g., French, Polish, Hungarian, Low German) an asset
